A Medicaid divorce lets the at-home spouse keep the house and up to $162,660 while Medicaid covers the nursing home's $9,277 monthly bill.
The strategy requires a real court-ordered divorce, since Medicaid's five-year look-back treats uneven transfers as gifts and penalizes them.
Before filing, families should exhaust spousal refusal, Medicaid-compliant annuities, and exempt-asset conversions, since a divorce strips survivor benefits and medical decision rights.

A Medicaid divorce, involves a long-married couple legally ending their marriage as part of a long-term care strategy. He enters a nursing home that Medicaid may cover, while she keeps the house and a larger share of their savings to protect her retirement. It's a real but uncommon strategy. State laws may vary, and many attorneys consider it a last resort.

Florida exempts a $600,000 IRA from Medicaid's $2,000 ***** et limit once the account enters payout status through RMDs or regular distributions.
Every IRA distribution counts as income against Florida's $2,982 monthly cap, and nearly all of it goes directly to the nursing home.
A named IRA beneficiary keeps the remaining balance out of probate, blocking Florida's estate recovery claim under Fla. Stat. § 409.9101.

A Florida retiree entering long-term care with $600,000 in her IRA looks like an automatic Medicaid denial. A single applicant there can't hold more than $2,000 in countable ***** ets. Thankfully, Florida's IRA Medicaid rules leave a narrow opening: once a retirement account is in payout status, the state can exclude the balance from the ***** et count. This means withdrawals still count, each distribution is treated as income, and that income ends up paying most of her nursing home bill.

The 'half-a-loaf' plan gifts roughly half of excess ***** ets to children, then uses a Medicaid-compliant annuity to bridge the penalty period.
Keeping retained cash in a bank account prevents the penalty clock from starting, since ***** ets above $2,000 disqualify the applicant outright.
One flawed annuity term or miscalculated divisor can unravel the entire plan, making elder law attorney guidance essential before any money moves.
Two retirees, same $1 million, same 4% rule, buy one finished with $1.4 million, the other hit $0 in 12 years. Our free reader guide explains the flaw that separated them, and the income-first method built to avoid it.
Picture a common scenario in a state where this type of crisis planning is welcome: Dad moves into a nursing home with roughly $200,000 sitting above the $2,000 countable-asset limit that most states use for single Nursing Home Medicaid applicants. His family gifts about $100,000 to the children, then uses the other half to buy a short-term Medicaid-compliant annuity that pays the nursing home while the gift penalty runs. When the penalty ends, Medicaid picks up the bill. The children keep the gift.

A home Medicaid excludes from **** et tests during life becomes fully exposed to estate recovery after death, potentially consuming most of an inheritance.
Transfers of a home within 60 months of a Medicaid application trigger a penalty period, and a stroke can eliminate the legal capacity needed to act.
Penalty-free exceptions exist, including spousal transfer, caregiver-child, and protected-relative rules, but qualifying circumstances must be in place well before a crisis.

An 82-year-old widower has a stroke. From the hospital he moves to rehabilitation, then to permanent nursing-home care. His paid-off house is worth $340,000. His daughter asks the elder-law attorney whether they can deed her the house or drop it into an irrevocable trust before Medicaid pays a dime. The answer is the one no family wants to hear: probably not, because Dad no longer has the capacity to sign, and his durable power of attorney authorizes bill payment but not gifts or trust transfers.

Without a signed, prospective contract, Medicaid treats family caregiver payments as gifts, exposing $60,000 in payments to a six-month nursing home ineligibility penalty.
A defensible care agreement must be signed before services begin, specify duties and market-rate pay, and be backed by time sheets and traceable payments.
At 20 documented hours per week, $2,500 monthly works out to roughly $29 per hour, defensible against the 2025 national median caregiver rate of $35.

Picture a widow in her 80s who pays her adult daughter $2,500 a month for two years to handle bathing, meals, medications, driving to appointments, and overnight supervision. The care is real. The daughter cuts back her own job to provide it. One year later, mom breaks a hip, lands in a nursing home, and applies for institutional Medicaid. The caseworker pulls five years of bank statements, spots $60,000 in recurring payments to the daughter, and finds no signed agreement, no time sheets, no invoices. In that state, the agency treats every dollar as an uncompensated transfer, effectively a gift.

The policy from President Donald Trump's administration, which took effect Friday, marks a dramatic change to a long-standing federal policy consistent with the president's efforts to restrict legal immigration pathways.
The rule could reduce federal and state payments by $13 billion a year, by persuading some 950,000 people to disenroll from or avoid federal safety net programs, according to estimates from the Department of Homeland Security. The agency said that the rule aligns with Congress's intent that noncitizens "in the United States be self-sufficient and not dependent on taxpayer-funded government benefits."

On August 11, Humana Inc. (NYSE:HUM) announced that its Medicaid plan, Humana Healthy Horizons in Indiana, is collaborating with HealthStream, Inc. (NASDAQ:HSTM). The partnership expands access to workforce training for caregivers, aiming to recruit and retain talent in rural and underserved communities. While one provides healthcare coverage and the other builds the software to train healthcare workers, both companies are capitalizing on the industry's critical focus: solving caregiving shortages and optimizing healthcare delivery.
HealthStream emerges as the operationally cleaner, higher-growth performer, while Humana continues to navigate a more complex transition marked by regulatory and profitability pressures. Humana Inc. (NYSE:HUM) reported Q2 2026 GAAP EPS of $5.73 and adjusted EPS of $7.61, with an Insurance segment GAAP benefit ratio of 91.2%. The company reaffirmed its full-year adjusted EPS guidance of at least $9.00 but lowered its FY 2026 GAAP EPS forecast to at least $6.52 from $8.36, reflecting non-cash adjustments and value creation charges. Although individual Medicare Advantage membership is expected to grow approximately 25% in 2026, lower Star Ratings remain a significant drag on profitability.
HealthStream, Inc. (NASDAQ:HSTM), meanwhile, delivered record Q2 2026 results, with revenue rising 12.5% year over year to $83.7 million. Operating income increased 41.4% to $8.3 million, while net income climbed 23.8% to $6.7 million, or $0.23 per diluted share. Adjusted EBITDA also increased 16.9% to $20.6 million. The company's debt-free balance sheet, supported by $66.7 million in cash and cash equivalents, further strengthens its financial flexibility. While Humana operates at substantially greater scale, HealthStream is demonstrating stronger operational leverage, margin expansion, and balance sheet flexibility.
Humana's bull case is supported by strong Medicare Advantage membership growth, with the company targeting a 25% increase in 2026, alongside strategic expansion of its CenterWell primary care business and state Medicaid footprint, including its recent Illinois win. These initiatives could strengthen its long-term recovery and expand its addressable market. However, the bear case centers on elevated medical benefit ratios, which reached 91.2% in Q2, as well as regulatory pressures in Medicaid and continued Medicare Star Ratings headwinds. These factors could further squeeze margins and weigh on net earnings.

S&P 500 net profit margins reached a decade-high of 14.8% in Q1 and are expected to remain above 14% in Q2 despite challenges like higher interest rates and geopolitical uncertainty. Technology remains a key focus in WestEnd's portfolio, along with investments in infrastructure, demographic shifts, financial innovation, and selective consumer opportunities. Also, check the fund's top five holdings to see its best picks in 2026.

Eagle Capital Management, an investment management company, released its second quarter 2026 investor letter. A copy of the letter can be downloaded here. In the quarter, Eagle Capital Management discussed how enthusiasm around AI capital spending has driven strong S&P 500 earnings growth while also increasing risks from elevated valuations, concentrated demand, and aggressive investment ******* umptions. Eagle remains a strong believer in AI but prefers constructing a portfolio that can perform across multiple outcomes rather than relying on one forecast. The firm believes current earnings can overstate underlying economics because semiconductor equipment is depreciated over several years, while free cash flow growth remains much weaker. It also expects competition and additional capacity across AI labs, hyperscalers, and semiconductors to eventually create winners and losers. These dynamics are encouraging Eagle to recycle capital toward attractive opportunities outside the most crowded AI trades while maintaining selective exposure to high quality beneficiaries. The portfolio trades at a 20% market discount with faster expected EPS growth. Please review the Strategy's top five holdings for key selections.

(NYSE:HUM), Elevance: These managed care companies have significant scale advantages in a consolidated industry that outgrows the overall economy. The industry moves with its own cycle and, over the last several years, has faced cost/price pressures in Medicare Advantage and Medicaid. Even the largest firms are earning poor margins, and some weaker firms are unprofitable. We believe conditions have bottomed out and that we are transitioning to a multi-year improvement in margins and earnings. Al-driven technology projects can both reduce costs and improve service quality. Humana is also making good strides, and we expect significantly improved Star ratings for the company later this year. We expect EPS growth of 15-20% over the next several years, driven by moderate revenue growth and significantly expanding margins."

Hospitals thought losing hundreds of billions in Medicaid funding through GOP-led cuts last year was bad. But coming regulations from the Trump administration could slash their funding even deeper than Congress did.
Two recently proposed rules from the Centers for Medicare and Medicaid Services would cost hospitals hundreds of billions of dollars more, hospital executives told POLITICO. The rules, which target taxes states use to get more federal Medicaid dollars — and which yield higher payments to hospitals — have prompted hospitals to launch another major lobbying blitz after record spending last year.
"Our core message to CMS is to stick to the statute. Congress cut enough," said Robert Nelb, director of policy at America's Essential Hospitals, which represents hospitals serving large Medicaid populations.
"There's no need to cut any more out of the Medicaid system at a time when the safety net is really struggling," Nelb added.
If the rules are finalized and they lose hundreds of billions on top of Congress' funding cuts, hospitals say they'll be forced to reduce services, lay off workers, consolidate operations or shutter entirely. Some systems are already doing so, telling POLITICO they've cut staff, ended contracts and slashed hospital beds in response to predicted losses.

A Florida grand jury found that Republican Gov. Ron DeSantis' administration "misappropriated" $10 million in taxpayer money it diverted to a charity connected to his wife, but it declined to file criminal charges due to a lack of evidence indicating who specifically was responsible.
DeSantis on Thursday did not dispute the legitimacy of the secret grand jury report obtained and published online by CBS News Miami, but he insisted no laws were broken and said whoever leaked the sealed documents would face "consequences."
The Leon County grand jury completed its report in January on investigations into Hope Florida, the charity started in 2021 by Florida first lady Casey DeSantis. Despite the lack of charges, the report ignited a new round of criticism aimed at DeSantis and other top state Republicans over the charity receiving $10 million from a state Medicaid settlement intended to help poor children get health insurance.
David Jolly, the Democratic nominee for governor, called for the grand jury probe to be reopened. He faces Republican Byron Donalds in November. DeSantis, under Florida law, cannot seek a third term.
The Hope Florida charity is supposed to help financially struggling families connect with churches and aid groups to help keep them off publicly-financed ***** istance programs. The $10 million was instead moved to political action committees that used the money to oppose a 2024 statewide ballot measure that would have legalized marijuana for adults in the state.

Minnesota Democratic gubernatorial candidate Amy Klobuchar is facing heat from her Republican opponent and critics on social media for campaigning on addressing the massive fraud scandal in her state despite not mentioning it a single time in any press release since the crisis emerged.
A Fox News Digital review found that Klobuchar's Senate office repeatedly issued press releases addressing other forms of fraud between 2022 and 2025, but a review of 1,000 of her official press-release archive found no release directly addressing or offering solutions to fix Minnesota's sprawling social-services fraud scandal or Feeding Our Future.
The $250 million Feeding Our Future scandal, that has ballooned into an overall social services scandal that prosecutors say could total over $9 billion, broke in late January of 2022. In more than 1,000 press releases from Klobuchar's office from January 1, 2022 to December 31, 2025, the Feeding our Future scandal was not mentioned in Klobuchar's releases, nor were the terms "Minnesota fraud," "social services fraud," "Medicaid fraud," "autism fraud," "Housing Stabilization Services," "child nutrition fraud," "DHS fraud," or "nonprofit fraud."
"As Feeding Our Future was breaking and all of the fraud that was being talked about, the absent voice that I was noticing was Senator Klobuchar," Minnesota Republican House Speaker Lisa Demuth told Fox News Digital in an interview on Friday.

On August 13, Abeona Therapeutics (NASDAQ:ABEO) reported second-quarter results that captured a company scaling a genuinely new kind of medicine while working through the ***** ps that come with it. ZEVASKYN, its cell therapy for a severe skin disease called recessive dystrophic epidermolysis bullosa, brought in $11.4 million in revenue for the quarter, and the number of patients treated kept rising. But the call also laid out cancellations, manufacturing hiccups, and a shift in how the company plans to report its own progress, all within the same three months.
Revenue climbed 31%, or $2.7 million, to $11.4 million from $8.7 million in the first quarter of 2026. Abeona has now treated 12 patients since ZEVASKYN's launch, including five in the second quarter and three more in the third quarter to date. The treatment center network grew alongside that, reaching seven activated sites with the recent addition of Cincinnati Children's, one of the largest epidermolysis bullosa treatment hubs in the country. New York-Presbyterian/Columbia University Irving Medical Center and Children's Hospital of Philadelphia also came online during the quarter, and CHOP moved fast, completing its first treatment in July just months after activating in May. UTMB, meanwhile, finished its first patient biopsy. Roughly 40% of the addressable patient population now has in-state access to a qualified treatment center.
Abeona also picked up a new technology add-on payment from the Centers for Medicare and Medicaid Services, effective Oct. 1, 2026, for fiscal year 2027, making it one of only three approvals out of 15 new applications that cycle. On the cost side, R&D spending fell to $5 million from $9.6 million, which had included a one-time $7 million licensing payment, and SG&A dropped to $15.8 million from $19.5 million. The company closed the quarter with $146.8 million in cash and short-term investments.
The net loss widened to $20.2 million, or $0.35 per share, from $17.1 million, or $0.30 per share, in the first quarter. Of the five patients treated in the second quarter, only four generated recognized revenue, because one batch's cell yield came in below the threshold required for recognition. Management pointed to ZEVASKYN's 84-hour shelf life as a core operational challenge, since it forces dermatologists, surgeons, anesthesiologists, and hospital staff to lock in exact dates well ahead of time. That rigidity showed up directly in the numbers: two scheduled biopsies were canceled at the last minute in the second quarter after patients' health unexpectedly declined, and because those slots had been booked so far in advance, they could not be filled by another patient.

HCA Healthcare, Inc. (NYSE:HCA) reported its second-quarter 2026 financial results on July 24, confirming the figures previewed earlier in the month. While top-line growth remained healthy, a visible deterioration in payer mix and a revision to full-year profitability guidance triggered immediate target cuts across Wall Street.
In its Q2 results, HCA Healthcare, Inc. reported that revenue increased 8.7% year-over-year to $20.23 billion from $18.61 billion in Q2 2025. The firm's net income attributable to the company rose 2.8% to $1.69 billion, while diluted EPS increased 11.6% to $7.62 (or $7.59 on an adjusted basis). Adjusted EBITDA grew 4.6% to $4.027 billion compared to $3.849 billion in the prior-year period. Operational volume remained positive overall, with same-facility admissions up 2.5%, equivalent admissions up 2.7%, and emergency room visits rising 3.6%. However, same-facility inpatient surgeries dropped 2.3%, and outpatient surgeries fell 3.4%.
Despite the top-line expansion, management was forced to adjust its full-year 2026 outlook downward. The company now expects 2026 diluted EPS of $28.70 to $30.50 (down from $29.10 to $31.50) and Adjusted EBITDA of $15.40 billion to $16.10 billion (down from $15.55 billion to $16.45 billion), while narrowing revenue guidance to $77.00 billion to $79.50 billion.
The primary culprit was a policy-driven payer mix shift: an uptick in uninsured volume following Medicaid redeterminations and the lapse of health insurance exchange coverage wiped out roughly $400 million from Q2 pre-tax income. HCA now anticipates the full-year exchange-related drag to reach $1.00 billion to $1.20 billion, partially offset by $300 million to $500 million in net Medicaid Supplemental Payment Program benefits.
Following the report, ***** ysts swiftly adjusted their models. On July 28, Mizuho lowered its price target on HCA to $475 from $525 while keeping an Outperform rating, citing slower post-Q2 growth expectations. The same day, Morgan Stanley reduced its price target to $380 from $425 and maintained an Underweight rating. Morgan Stanley ***** yst noted that while lower guidance "puts numbers in a better place," core EBITDA performance was "disappointing," warning of a full valuation alongside a rising risk profile in payer mix.

The Trump administration on Tuesday finalized a rule that will bar Medicaid from covering gender identity healthcare for children enrolled in the government's low-income insurance program.
The new policy prohibits states from using federal Medicaid funds to pay for any gender identity care for enrollees under 18, marking the administration's latest effort to restrict youth access to transgender healthcare.
President Donald Trump celebrated the move in a Truth Social post, touting it as necessary to crack down on "barbaric surgeries and practices, which result in unthinkable and irreversible harm to their young bodies."
"Just think about all the young, innocent, and perhaps confused children who will be spared!" Trump wrote on social media.
The president has sought to make transgender care a central issue in the run-up to the midterms, frequently describing such gender identity healthcare procedures in graphic terms and claiming, without evidence, that swaths of children are being pressured to secretly transition without their parents' knowledge.

Equity markets posted strong returns in the second quarter of 2026, with the Russell Midcap Index rising 13.8% and the Russell Midcap Value Index returning 13.4%, despite concerns about inflation, a hawkish Federal Reserve, and rising oil prices due to the Iran conflict. Narrow market leadership was evident, particularly semiconductor stocks and other stocks in the AI sector, which saw returns exceeding 100%. The Firm favors quality businesses with attractive valuations, believing that fears regarding AI's impact are overstated. The Hotchkis & Wiley Mid-Cap Value Fund lagged the Russell Midcap Value Index, achieving a 4.74% return in the second quarter, primarily due to underperformance in technology and energy sectors, while stock selection in healthcare contributed positively. In addition, please check the Fund's top five holdings to know its best picks in 2026.

The Fund gained 11.99% during the quarter, compared with the 10.48% gain for the Russell 3000 Health Care Index and the 15.44% gain for the Russell 3000 Index. Since inception, the Fund appreciated 10.61% on an annualized basis, compared with 10.02% for the Benchmark and 14.83% for the Index. Strong stock selection in pharmaceuticals, biotechnology, health care equipment, and life sciences tools and services supported the Fund's outperformance, although limited exposure to managed care stocks reduced relative returns. The Fund remains positive on health care due to improving biotechnology funding, strong acquisition activity, recovering managed care margins, and growth from an aging population, chronic disease, medical innovation, and higher health care spending. In addition, please check the Fund's top five holdings to know the best picks in 2026.
In its second-quarter 2026 investor letter, Baron Health Care Fund highlighted Elevance Health, Inc. (NYSE:ELV). The fund re-established a position in Elevance Health, Inc. (NYSE:ELV), a US-based health benefits company, during the quarter. On August 03, 2026, Elevance Health, Inc. (NYSE:ELV) closed at $382.77 per share, reflecting a market capitalization of $81.54 billion. Elevance Health, Inc. (NYSE:ELV) posted a one-month return of -8.61%, while its shares gained 38.48% over the past 52 weeks.
Baron Health Care Fund stated the following regarding Elevance Health, Inc. (NYSE:ELV) in its Q2 2026 investor letter:
"We re-established positions in two previously owned managed care companies, UnitedHealth Group Incorporated and Elevance Health, Inc. Both companies manage diversified portfolios, providing insurance and health care services to Commercial, Exchange, Medicaid and Medicare Advantage members. We believe that the insurance cycle is turning more favorable for these companies, particularly in their Medicare Advantage businesses. After several years of elevated utilization trends, inadequate reimbursement, and regulatory challenges coupled with aggressive pricing to drive share gains, which drove operating margins to depressed levels, UnitedHealth and Elevance have exited unprofitable Medicare Advantage markets and products, right-sized benefits, and are now in the process of rebuilding profitability. We further think that the application of AI will enable them to take a significant bite out of administrative costs as well. Finally, we believe that the earnings power of both companies is well above current levels ******* uming they can approach their longterm target margins over the next few years. If we further ******* ume a reasonable multiple on future earnings power, we believe there is substantial upside in both stocks."
